How Forms Academy collects, uses and shares personal information about our families, prospective families and visitors to www.formsacademy.com.
Collection of personal information
Information collected directly from you. We may collect personal information directly from you, for example through a web form, during registration, while booking an evaluation or open session, while setting up an account with us, when you contact us for support, or at checkout. This may include first and last name, address, email address and phone number. When a parent or guardian registers a player or books a session, we also collect the player's name and date of birth.
Information collected from your device. Our website may use cookies, web beacons, pixels and similar technologies to automatically collect certain information from your device, such as your IP address, the date and time of your visit, browser and operating system information, the referring website address, and how you interact with the website. Our email campaigns may also use tracking technologies to collect information such as your IP address, browser type and version, and email engagement statistics.
Information collected from our partners. We may collect personal information about you from third-party partners and combine it with other information that we collect.

Forms on this website
Contact, careers, open session and waitlist forms on this website are processed by our website host, Netlify. Open session and evaluation requests are also sent to Forms Academy's enrollment system so our staff can confirm your session. Registration and tuition payments are handled at start.formsacademy.com, and card payments are processed by Stripe. We do not store full card numbers.
Disclosure of personal information
We may share the information we collect with other parties, including:
- Third-party service providers that help us provide and market our programs, such as payment processors, data hosting, analytics, online store support, advertising partners and email service providers;
- Constant Contact, to send email marketing and text message communications. See Constant Contact's Customer Data Notice;
