Program and website terms.
Tuition, fees, cancellation, the Forms 30-Day First Touch Guarantee, the referral program, and our mobile messaging terms, in one place.
These terms summarize how Forms Academy LLC ("Forms Academy", "we", "us") runs its Academy programs and this website. If you sign an enrollment or program agreement during registration, that agreement governs where it differs from this summary.

- Card processing fees are passed through at checkout, so the amount charged is slightly above the listed tuition.
- Registration: $99 per player per year, waived if you enroll within 24 hours of your complimentary evaluation.
- Facility fee: $250 twice per year, in October and April, covering field maintenance, equipment and insurance.
- Futsal: $100 per month when bundled in Fast Track, $125 per month when added separately.
- Uniforms and league fees are billed separately.
Contracts and cancellation
Monthly Academy plans have no contract and no cancellation fee. Cancel at any time with 30 days notice. You can upgrade or downgrade your plan at any time, and changes take effect at your next billing cycle.
The Forms 30-Day First Touch Guarantee
Train twice a week for 30 days and make at least 7 of your 8 sessions. If you do not see real improvement in your player's first touch and speed of play, tell us and your next month of tuition is credited in full. You are the judge.
- The guarantee applies to new enrollments on the Accelerator and Fast Track plans, which train twice a week, and it covers the player's first 30 days.
- The player attends at least 7 of their 8 scheduled sessions in the 30 days.
- There is no test. The family decides whether they have seen real improvement and tells us.
- The credit is applied to the next month of tuition in full. It is a tuition credit rather than a cash refund.
Referral program
- When a family you refer enrolls in the Academy, your account receives a $175 credit. For a Residency enrollment, the credit is $500.
- The credit is applied after the referred family completes their first paid month.
- The referred family's $99 registration fee is waived.
- Credits apply to tuition and are not paid out as cash.
Mobile messaging terms
Forms Academy operates a mobile messaging program (the "Program") subject to these Mobile Messaging Terms. The Program and our collection and use of your personal information are also subject to our Privacy Policy. By enrolling, signing up or otherwise agreeing to participate in the Program, you accept these Mobile Messaging Terms and our Privacy Policy.
1. Program description
We may send promotional and transactional mobile messages through the Program. Promotional messages advertise our programs and services and may include promotions, specials and other offers. Transactional messages relate to an existing or ongoing transaction and may include registration notifications and updates, session reminders and other transaction-related information. Messages may be sent using automated technology. Message frequency will vary but will not exceed 5 messages per month. You agree that we, our affiliates and our service providers may send you messages on these topics and that such messages may come from different telephone numbers or short codes. We do not charge for messages sent through the Program, but message and data rates from your mobile provider may apply.
2. Opting in
By providing your mobile phone number to us, you opt in to the Program and agree to receive recurring messages at that number, even if it is registered on a state or federal Do Not Call list. You confirm that you are the owner or authorized user of the number. If you change your number or are no longer its owner or authorized user, please tell us promptly at admin@formsacademy.com. Participation is not required to make any purchase and is completely voluntary.
3. Opting out and support
You may opt out at any time by replying STOP, QUIT, CANCEL, OPT-OUT or UNSUBSCRIBE to any message from us. You may receive messages for a short period while we process your request, and you may receive a one-time confirmation. For support, reply HELP to any message. Our platform may not recognize requests that change these commands, and we and our service providers are not liable for failing to honor requests that do not follow them. If we change the number or short code we use, we will notify you, and we are not responsible for requests sent to a number or short code that has been changed.
4. Disclaimer of warranty and liability
The Program is offered on an as-is basis and may not be available in all areas, at all times or on all mobile providers. Neither we nor our service providers are liable for any failed, delayed or misdirected delivery of any message or information sent through the Program.
5. Modifications
We may modify or cancel the Program or any of its features at any time, with or without notice. To the extent permitted by law, we may also modify these Mobile Messaging Terms at any time, effective when posted on this website. Your continued participation in the Program means you accept those modifications.
