School and training in one day.
Forms Residency is the Cognia accredited private school of Forms Academy, built for student athletes. Academics and two daily training sessions sit inside one school day, under the same FORMAX methodology as the Academy.
One accredited institution, two campuses.
The methodology is identical. The environment matches the age.
Primary Residency, grades 1 to 6, is hosted at the Z-Plex, 3059 Champions Way, Melissa, Texas. Academic and athletic environments share one facility, so the day moves between classroom and pitch in minutes.
Upper Residency, grades 7 to 12, is hosted at the INPOWERiQ Performance Center in McKinney, Texas. Academics expand to college-bound rigor and training intensifies for high-performance competition.
On June 27, 2024 the Cognia Global Accreditation Commission awarded Forms Academy Residency accredited status through NCA CASI, NWAC and SACS CASI, for a six-year term. The accreditation is held by Forms Academy as an institution and applies across its locations.
Academy players are eligible for the Residency, and the Residency connects to Forms Madrid, the international pathway in Alcobendas, Spain.
Texas Education Freedom Accounts, accepted here.
Forms Academy Residency is an approved TEFA vendor. Eligible Texas families can apply a state funded education account to tuition, fees and approved educational expenses at Forms.
A state funded education account
TEFA funds an account for an eligible student. It can pay tuition and fees at approved private programs such as Forms Residency, as well as approved tutoring, educational therapies and materials.
Texas students eligible for public school
The student must be a U.S. citizen, national or lawfully admitted resident, live in Texas, and be eligible to attend a Texas public school or open enrollment charter school.
Priority tiers, not first come
If eligible applications exceed the funding available, the state awards accounts by priority tier, beginning with students with disabilities and lower income households. Applying early does not give funding priority.
A TEFA award covers part of Residency tuition. Your family pays the remaining balance. Talk to us and we will walk through what the Residency would cost your family with an award.
The steps
- Apply to Forms. The Forms application and the TEFA application are separate. Securing a Residency seat first means you are ready to enroll if funding is awarded.
- Tell us you are pursuing TEFA, so we can keep you informed through the award notifications.
- Submit your TEFA application through the state portal during the application window. The 2026-27 window ran February 4 to March 17, 2026. The state sets the dates for each year.
- If you are awarded, confirm with us. Forms reports to the Texas Education Agency so the funds can flow to your tuition account.
Full TEFA details, eligibility documents and the disability priority forms are on formsresidency.org/tefa. The state program is at educationfreedom.texas.gov.
